Resource Coordination is a process whereby an individual and/or family’s needs are identified and LHDC services are coordinated in a systematic way. LHDC Resource Coordinators are available to you to help guide you and develop a plan for your personal needs.
LHDC Resource Coordinators will meet with an individual/family to assess needs. Afterwards, a plan of action is developed with the individual/family. They will oversee implementation of the plan by following up with the individual/family at regular intervals. Regular intervals will vary throughout the different stages of the plan. Resource Coordinators will work with individuals that can benefit from assistance with housing, income and employment, access to healthcare, support systems, education, linkage to transportation, food, and moving from crisis stabilization to success planning. Our hope is that individuals and/or families will build a relationship with a Resource Coordinator that will empower them with the tools they need to become self-sufficient.

Resource Coordinators are available in Perry, Spencer, and Crawford Counties.
